What ages does the Clinic treat?
Care is provided for the whole family—12 months and up (24 months and up in Kentucky). Patients 18 years old and under must be accompanied by a parent or guardian. Ages for specific vaccinations and other services may vary.

What services does the Clinic offer?
We diagnose and treat common illnesses and injuries including; allergies, colds, sore throat, ear and sinus infections and more. The Clinic provides a wide range of quality and affordable family healthcare services to help you prevent, treat and manage your healthcare concerns. We also offer a full range of vaccines and physicals including; camp, sports, school, DOT and life-event.

What information should I bring for a sports or camp physical?
Please bring a copy of the patient’s immunization records. It is also helpful to bring the necessary forms provided by the school or organization already filled out to save time. The Clinic has generic forms that your school or camp may accept if you do not have the necessary forms.

Please note: Consent from a legal guardian is required before treating a child under the age of 18 who is accompanied by an adult who is not the legal guardian. Sports and camp physicals are not covered by insurance.
